In person or virtual visits are offered.

Patients must be in Washington state for virtual visits.

I do not accept patients that are insured by Medicare.

New Patients

Upon creating a NEW PATIENT appointment, you will receive an invitation to CHARMHealth. Please sign up and fill out onboarding documents at least 72 hours prior to your first appointment. See more information and how to prepare for your visit HERE

All patients are required to be established with a primary care provider (PCP)

Elizabeth Goldberg PhD ARNP does not serve as a primary care provider and does not take after-hours calls. 

If you have an urgent medical concern, please contact your PCP, go to an Urgent Care or Emergency Department, or call 911.

CANCELLATION

A credit card on file is required to book an appointment. If you cancel your appointment with less than 48 hours notice, you are liable for a cancellation fee of $200 for new patient appointments or $100 for follow up appointments and your credit card on file will be charged.

Discovery Call     (15 minutes)  FREE
New Patient visit (60 minutes) $400
New Patient visit (90 minutes) $600
Established visit  (30 minutes) $200
Established visit  (60 minutes) $400
Extended time     (15 minutes)  $100

Payment

I do not accept or bill insurance. Payment is due at time of service. A receipt of payment or superbill may be submitted to your insurance company for out-of-network benefits.

Payment accepted: credit card,
cash, check and HSA.

Fees do not cover lab testing.

Prepare for your New Patient appointment
Schedule your first appointment HERE  Fill out intake forms before your first appointment. Some of the intake forms are lengthy. This thorough intake saves us time in the initial appointment. Upload all relevant medical records into CHARM. Fill out new patient forms at least 72 hours prior to your first appointment.

We may begin with orders for any recommended testing and/or develop a personalized plan to address your specific needs right away.

Complex health concerns
After the initial visit, I’ll create a personalized plan to address  your concerns – no guessing. At the follow-up, we’ll review the plan together and make any adjustments based on your needs.

How long does it take to restore balance and see improvement?
The timeline varies. If the issue has developed over years, it will take time to achieve a lasting balance. Commitment, patience and consistency are key to success.
